Zunächst wurde dazu das Krankenhaus als Wirkungsstätte des OP-Managements charakterisiert und insbesondere die Notwendigkeit eines wirtschaftlichen Handelns als Folge des seit dem 01.01.2004 eingeführten DRG-Systems thematisiert. Zudem erfolgte die Darstellung der funktionsorientierten und hierarchischen Aufbauorganisation eines Krankenhauses.
Die examine krankenhausinterner Prozesse aus wertschöpfungs- und patientenorientierter Perspektive legte dar, dass die invasive Therapieform des Operierens zu den Kernprozessen eines Krankenhauses zählt und infolgedessen eine hohe Bedeutung hinsichtlich der strategischen Handlungsdeterminanten Kosten, Zeit sowie Qualität besitzt. Des Weiteren wurden bauliche Gegebenheiten von Operationssaaleinheiten dargestellt und resultierende logistische Herausforderungen hinsichtlich notwendiger Güter- und Informationsflüsse abgeleitet. Die Darstellung resultierender Problemfelder aus dem Zusammenspiel der Teilprozesse ohne eine zielgerichtete Koordinierung begründete den notwendigen Einsatz eines OP-Managements im Operationsbereich.
Dementsprechend wurden zunächst aus den Grundzügen des Krankenhausmanagements die Zielstellungen eines OP-Managements bestimmt, um anschließend in Analogie zur klassischen Produktionsplanung/ -steuerung eines herstellenden Unternehmens die Handlungsfelder aufzuzeigen: Planungsphase (OP-Statut, Programmplanung, Material-/ Personal-/ Kapazitätsplanung), Realisationsphase (Umsetzung, Planänderung) und Kontrollphase (Dokumentation, Qualitätsüberwachung, Controlling).
Die an ein systematisches OP-Management anknüpfenden Möglichkeiten der Prozessbeschleunigung und einer erhöhten Mitarbeiterdisziplin/ -zufriedenheit stehen bei deren Umsetzung jedoch Einschränkungen in struktureller, personeller und organisatorischer Hinsicht gegenüber. Auf Grund der partiellen Beeinflussbarkeit dieser Rahmenbedingungen lassen sich für das OP-Management verschiedene Verbesserungspotenziale entwerfen(Neugestaltung/ -ausstattung des OP-Traktes, erweiterte Nutzung medizinischer Therapiepfade, Einführung eines Modul-/ Kanbansystems für Bereichslager). Eine abschließende Betrachtung des OP-Management-Konzepts verdeutlichte, dass die erzielbaren Erfolge durch eine gesteigerte Prozessfluss- und Patientenorientierung noch verstärkt werden können.
